How do I schedule an appointment?
You can schedule your first and all follow up appointments online.
What do I need to do prior to my appointment?
You will need to complete your intake forms online prior to your appointment. After scheduling your initial evaluation, you should receive an email with a link to your intake forms.
What should I bring to my first visit?
- Driver’s license or government issued ID
- Any lab or imaging results
- Comfortable clothing to easily expose the regions that will be looked at
Please arrive a few minutes prior to your appointment time to allow for starting on time. We value everyone’s time and work hard to start on time for each appointment.
What should I expect at my first visit?
Your initial evaluation will last approximately 1 hour. The evaluation will include a review of pertinent medical history, a series of questions during the subjective examination to better understand your pain and concerns, assessment of the muscles and joints related to your affected area, strength and neurological testing, movement and posture assessment, and if time, a trial treatment.
When is payment due?
Because we are out-of-network payment will be due at time of service.
In order to not take away from valuable treatment time, your credit card will be securely stored in our intake system. You card will not be charged until after your appointment.
All major credit cards are accepted forms of payment. You may also pay with cash or a check at the time of the visit, but a card will still be held on file.
What is the cancellation policy?
We require 48 hours notice if you cancel or reschedule an appointment otherwise you will be charged for a full session. You will receive an email or text reminder prior to your appointment. For some circumstances the fee may be waived, but this will be on a case-by-case basis.
